What dilemma did Sadao and his wife face when they saw the wounded American soldier?

Dr. Sadao found an American soldier badly wounded. As a doctor, it was his duty to save the dying man, yet he was in a dilemma as the American soldier was an enemy of his country, Japan and it was his duty to report to the army about the soldier. If they helped the stranger they would be treated as traitors. If he was thrown back into the sea, he would surely die, which would be against the ethics of their profession.
